Captive Portal not let me login

hi all :)
my Captive Portal not let me login
also if i set the Authenticate using to nothing selected
it always respond: login failed
what can i do?

i change  the temple to none because login and opnsense old logo no show on the page
can you give me example of HTML code for the new version ?
my machine:
1) physical brand computer
    I3-4130 CPU @ 3.40GHz (2 cores, 4 threads)
    8GB Ram
    120GB SSD
    3X1GB Nic

Run: captiveportal, ClamAV Daemon, Insight Aggregator, Intrusion Detection, OpenVPN server, ACME client, zenarmor.
