Is the clam av plugin useful for anything other than when used with a proxy?

also for a local nextcloud (this is a bad idea btw.) running with the nginx plugin.

Do you mean it's a bad idea to run nextcloud on opnsense? Not running nextcloud behind opnsense on its own server.

Running it directly on the OPNsense of course. The plugin would be capable of doing this but you open a huge attack surface. It is no problem to run your PHP applications behind OPNsense on a separate server and proxy the requests (using one of HAProxy, nginx, relayd) to it.
