Can't install OPNsense: FAILED with a return code of 1

I'm trying to install OPNsense, I've logged in as "installer" with "opnsense" as password. I then choose the "guided installation", select the disk "ada0", then "GPT/UEFI mode".

Execution of the command
/sbin/gpart create -s gpt ada0

FAILED with a return code of 1.


I've also tried the "manual instllation" > "install OPNsense" > select the disk "ada0" > "format this disk". Accepting all the defaults. But end up with the same error as above.

What is the issue?

The Qotom box came with OPNsense preinstalled. So I thought the reason for the installation failing, was because I was booting from the SSD and not the USB. But I've changed the boot option order in the BIOS to my USB as number 1 and the SSD is number 2. But I can't verify where OPNsense is booting from. I don't know what return code of 1 means. Is due to a partition? How can I check that? It SSD doesn't seem to be partitioned.   

I can't image that it is a hardware problem.

Can anyone help? Ideas on what to try?

SSD appears to be on 1 not 0, try using ada1. I use a couple of Qotoms, but it looks like yours is a newer version by the 2020 in the Bios. Never had any problems with them, both are rock solid.

You are properly right.
I just ended up erasing the entire SSD. And everything worked out smoothly.
