Issues with Port Forwarding

Hi all,
Enjoying my time with opnsense but I've followed so many tutorials and tried so many solutions to getting ports forwarded to no avail.
I've got bridge modem -> opnsense -> cisco l3 switch -> network
opnsense is running Unbound and I've got an external pihole that it talks to successfully.
OPNsense 21.1.5-amd64

The last walk-through i tried

I'm starting to suspect my ISP is messing with me at this point and considering purchasing my own modem just to completely rule out the equipment I can control.
Only other thought is would pihole be messing with it? Currently I'm Gateway opnsense IP and DNS pihole IP.
Let me know if I missed anything, I'll be happy to provide more information for any assistance
