Why are you confused about the pfsync interfaces?
They have nothing to do with carp. You need it for the config and state sync, not for carp.

I wasn't sure that the pfsync interface could somehow affect СARP, if it doesn't affect anything then the problem is somewhere else.


Another thing I noticed is that when i enable NAT for the LAN interface so that local users can access the Internet, and specify the VIP WAN address in the NAT Address field, my clients practically have no access to the Internet, it is very slow, when executing the ping command to 8.8.8.8, packets are dropped, in general the connection is not stable.
When i change the NAT address to a physical WAN, everything works fine, maybe this is also related to CARP, I don't understand it yet.

I have a physical Opnsense DEC750 and another Virtual Machine with OpnSense on (ESXI), I configure HA according to the manual: https://docs.opnsense.org/manual/how-tos/carp.html

The Master can synchronize with the Backup, they see each other, but at the moment when the Master fails, the Backup does not take responsibility and does not act as the Master.

My interfaces look like this:
