Last update for me on this topic;
Disabling IPv6 is the workaround which is a poor fix for this issue.

I got yet another LTE modem and after analysing the the traffic with TCP dump I gave up on trying to solve why it fails it just fails. If both IPv4 and IPv6 are enabled on WAN, the interface accepts only IPv6 address from DHCP and discards offered IPv4 address.

Since this is a site with only LTE connection available I lost all connectivity, yes.
I did now buy another LTE modem and with that one I was able to get even 22.1.6. to work. So the issue is with the interfaces and compatibility with the Netgear LTE modem for me.

Could someone at OPNsense have a look at changes from 22.1.4 that might affect the issue at hand?

I've now tried with completely different hardware for the OPNsense box and the issue persists. My suspicion is that my WAN device which is a Netgear LB1111 isn't handshaking properly. The strangest thing though the link is up just not able get the Gateway online, all of this works in 22.1.2 without any issues. Just updating to later builds brakes this. I've now ordered another LTE modem and will try with that to pinpoint the issue.

After trying all the tricks of rebooting and such without success I did reinstall OPNSense.

22.1.2 works without a problem. I then upgraded again to 22.1.5 and same thing occured. No way of getting the gateway up. I get both IPv4 and IPv6 addresses by the ISP's DHCP but something is severely broken in the system and no traffic is routed. The routing table looks okay. So only mitigation to this was a clean install again.

Another thing I noticed during this is that if you do not have the latest build available you cannot install packages, for example dyndns client can't be installed on an slightly older version and I'm now missing that package since I can't upgrade due to issues mentioned.

I recently upgraded my systems to 22.1.5 and one of the opnsense machines lost gateway access after the upgrade.

I have tried all the tricks in the book without success of solving the issue at hand.

I would have reverted the update but that needs internet connectivity that I don't have due to reason mentioned before.

Seems like there's some cache or conf file containing wrong information I'm guessing. Or do I have make a clean install and hope for the best?
