Under System->Settings->General where you entered the DNS servers, did you set the correct "Use Gateway" for each server?

Also, did you renew the client DHCP after making your OPNSense modifications?

You need to resolve the issue with correctly burning the ISO. Did you confirm the checksum matches after downloading the image? If the checksums do not match, your download is bad.

Another option is to disable NTP and install the Chrony plugin.

On the switch between OPNSense and your server. That said, what is the point of running LAGG to OPNSense? Do you have a 10Gig+ WAN connection?

Thank you for providing the hardware inventory. Are you able to connect via the serial interface? Have you tried connecting a monitor to see what is being displayed when the LAN is reconnected?

I simply added the DNS servers I wanted under System: Settings: General, setting the correct interface (WAN_DHCP/WAN_DHCP6) depending on the address entered (IPv4/6).

Why is bridging not recommended? I've read elsewhere, more than once, that it's to be avoided, but I'm not sure why. Is it based on the assumption that the hardware isn't up to the task?

Performance can suffer when using a bridge because the firewall's CPU has to handle all of the traffic passing between systems on the two (or more) bridge ports. Using an ethernet switch eliminates that bottleneck and let's the switch handle all intra-LAN traffic.

Connecting to your switch would be a better option. Move your existing LAN connection to the switch along with the AP.

My guess is the disk is bad and needs replacing. You can try a reinstall, but I expect the problems to return.

What is the hardware you are using for the servers? Also, brand and model of the drives? Are you using a raid controller?

Those zfs errors indicate either a bad disk or a bad connection to the drive. What SMART tests did you run? I would try reseating the cables connecting the disk for a start.
